License Renewal

Certified Nursing Assistants (CNA) must renew their Certification every two (2) years. We CANNOT sign off on your renewal form, only facilities where care is actually being given can sign off on your forms. Any facility that you worked at least 8 hours within the past two (2) years can sign off on your form. We also CANNOT request an extension, if you require an extension ONLY a facility can request an extension and you can only work at the facility for the time of the extension.
