• Current License good for 2 years
• PPD test (within the past year) or evidence of chest x-ray (within the past 3 years)

• Social Security Card
• CPR Card good for 2 years
• Recent Physical Exam 
• Rubella & Rubeola screen (MMR) Titers Blood Test 
• Varicella (Chickenpox) Titers Blood Test 
• Hepatitis B vaccination Titers Blood Test 
• Criminal Background Check **
